US Dept of Health & Human Services / Center for Medicare & Medicaid Services LogosThe address and telephone number presented on a Provider-Page is the Primary Practice Address published by the U.S. Department of Health and Human Services (HHS), Centers for Medicare and Medicaid Services (CMS). National Provider Identifier (NPI) numbers and registrant information are public records available for download at the CMS website. Many companies present NPI information on the Internet.

To view your record in the National Plan and Provider Enumeration System (NPPES) visit https://npiregistry.cms.hhs.gov/search.

You can change the information presented on your Provider-Page by CLAIMING your page and updating the practice address. Editing Practice / Organization Name, Address, Main / Appointments Telephone will prompt you to provide certain information to validate your identity. A confirmation will be sent once your profile is corrected. Please allow 72 hours to update our system.

US Dept of Health & Human Services LogoDr. Leonardo obtains data about healthcare providers from U.S. government and other public databases. If your information is inaccurate, you can correct or update the listing by CLAIMING your Provider-Page and updating your profile.

You can view your record in the National Plan and Provider Enumeration System (NPPES) by visiting https://npiregistry.cms.hhs.gov/search.

Remove Your Provider-Page ListingDr. Leonardo will gladly remove your Provider-Page. Simply CLAIM your listing and navigate to Your Profile. Select Delete Doctor Profile. You will be prompted to provide certain information to validate your identity and ensure that your Provider-Page is not removed maliciously.

A confirmation will be sent once your profile is removed. Kindly allow 72 hours to purge it from our system.

No Phones IconYour Provider-Page is designed to look great on mobile devices, and editing your listing using a desktop or laptop is a cinch! Unfortunately, you are not able to edit information using a mobile device at this time.

Locked IconVisitors are prevented from accessing a Provider-Page and changing certain profile information maliciously. To correct your name, you must first CLAIM your listing and navigate to Your Profile. Selecting First Name, Last Name or Middle Initial will prompt for certain license information for validation purposes.

A confirmation will be sent once your profile is corrected; kindly allow 72 hours to update our system.

Locked IconDr. Leonardo requires validation when changing address information on a Provider-Page. CLAIM your listing and navigate to Location Info. Selecting Practice / Organization Name, Address, Main / Appointments Telephone will will result in an email that will request certain license information for validation purposes.

A confirmation will be sent once your contact changes are approved; kindly allow 72 hours to update our system.

Clock IconYes — CLAIM your Provider-Page and navigate to Location Info. Click on the tab Office Hours to update the days and times that you are open.

Schedule Your Appointment ButtonYour Provider-Page features a native scheduler that captures a visitor’s name, email, phone, date and time of a requested appointment. Patients are advised that this is not a direct scheduler and that they must call to confirm their requested date and time for an appointment.

The feature can be toggled ON or OFF. If the scheduler is turned ON, just provide an email address to where appointment requests are to be sent.

CLAIM your Provider-Page and navigate to Online Scheduling. Toggle ON or OFF. If ON, enter an email address.

Update Your Portrait PhotoSimply CLAIM your Provider-Page and upload your portrait photo. You can also indicate whether you are accepting [new] patients.

Add Your Practice, Health System, or Hospital NameYes. CLAIM your Provider-Page and navigate to Location Info. You can also edit the Slogan / Discipline.

Default Provider-Page LogoAdding a custom logo requires a subscription. CLAIM your Provider-Page and SUBSCRIBE to a Provider-Site. Visit Pricing for more information.

Design Theme Color PickerCertainly! CLAIM your Provider-Page and navigate to Design Elements / Color Schemes.

Provider-Page Default Banner ImageA great banner image on your profile is guaranteed to make your page stand out! Dr. Leonardo accommodates custom photos and illustrations. Changing the image requires a subscription — CLAIM your Provider-Page and SUBSCRIBE to a Provider-Site. Visit Pricing for more information.

Health Information IconDr. Leonardo has selected Health Information topics associated with your health discipline. You can change any of these topics by viewing our Patient Education Library of 7,000+ health topics sorted by specialty. A maximum of five (5) articles can be presented on your FREE Provider-Page. CLAIM your Provider-Page and navigate to Patient Education / Health Information to change content.

Provider-Sites subscriptions can present as many Health Information topics as you like. Each can edited, and you can have an unlimited number of additional web pages as well. CLAIM your Provider-Page and SUBSCRIBE to a Provider-Site. Visit Pricing for more information.

NOTE: Patient Education / Health Information content has a significant effect on search positioning, and works to get you ranked for broad search terms related to your specialty!

Login to EHR Patient Portal ButtonThe button [Login to EHR PATIENT PORTAL] only functions on Provider-Site subscriptions. On a CLAIMED Provider-Page (free version), you can prevent the button from displaying on your page — CLAIM your Provider-Page and navigate to Profile > Patient Portal EHR. Toggle to OFF.

Google IconYour Provider-Page can automatically feature reviews from Google and other third-party services such as Rater8, Ratings.MD, and others. A subscription to a Provider-Site is required. Visit Pricing for more information.
